The customer fills out the test application form or submits an equivalent document previously prepared, such as the Sample Acceptance–Transfer Act.

Information needed to design an order:

• the name, legal address, properties of the contracting party.
• the name and phone number of the contact person.
• the name and location of the object (address).
• type of sample (e.g., monolithic, mineral material, sand, gravel, etc.).
• sampling plan (date of sampling, performer, point of sampling, drilling No, depth, etc.).
• determinable parameters (in coordination with the laboratory).
